REQUIRED DETAILS
- Application form (version 5.5).
- If the customer is not ready for video verification ,Submit DRAFT letter.
- Address proof: Customer Aadhar (Attested copy) Mandatory.
- Identity Proof: Pan/ Driving License / Passport (Attested copy) Mandatory.
- Authorized person’s government ID ( Self Attested copy).
INSTRUCTIONS
- Application form filled with Blue / Black ink and Capital letters.
- Photo must be affixed in the application form and it should be cross signed.
- Office seal , Customer PEN number & Aadhaar Number is mandatory in application first page .
- Authorized person’s seal & signature is mandatory in the Draft letter.
